Life Coaching Services Acknowledgment & Self-Pay

Nature of Services

I understand that the coaching services I am receiving from Paul Doherty are NOT psychotherapy, mental health treatment, or clinical counseling. No diagnosis or treatment of mental health conditions will be provided.

Separation from Counseling Practice

I acknowledge that coaching is a separate and distinct service from counseling. While Paul Doherty is a licensed counselor, these coaching sessions are not being provided under his license and are not subject to HIPAA or clinical documentation practices.

Payment Terms

I agree to pay a flat rate per coaching session. Payment is due at the time of booking service. I understand that these sessions are not eligible for insurance reimbursement and that I will not submit claims or seek coverage for them.

Confidentiality

Coaching services are not bound by HIPAA regulations. Confidentiality will be upheld to the highest extent, but these are not protected health services.

Voluntary Participation

I am choosing to participate in coaching services voluntarily and understand the scope and limitations of these services. I understand I may discontinue at any time and may be referred to licensed therapy services if clinical issues arise.
